Прошу прийняти мене до членів ГРОМАДСЬКОЇ ОРГАНІЗАЦІЇ "МОЛОДЬ ЛЬВОВА".I hereby apply for membership of NON-GOVERNMENTAL ORGANIZATION "YOUTH LVIV".

Зі Статутом організації ознайомлений (-а), зобов’язуюся дотримуватися його положень, сприяти досягненню мети організації та виконувати рішення її керівних органів.I have read the Statute of the organization and undertake to comply with it, to help achieve the organization’s purpose and to comply with the decisions of its governing bodies.

Підтверджую, що досяг (-ла) 14 років і є громадянином (-кою) України або іноземцем чи особою без громадянства, які перебувають в Україні на законних підставах.I confirm that I am at least 14 years old and that I am a citizen of Ukraine, or a foreign national or stateless person lawfully staying in Ukraine.
